Category of your grievance (whether content-related or classification/ age-ratings related)
Programme details (including the relevant title, season and episode of the programme and timestamp of concerning content)
Country of viewing
Date of viewing
Brief summary of your grievance

Any content-related grievances will be addressed within 15 days from the date of receipt of the grievance.
